This provides a line of defence for the lungs from invading bacteria, viruses, and other foreign substances. It is a slippery, clear, water-based solution which contains enzymes, antibodies, and proteins. In essence, this means when you breathe in, the mucous is a kind of safety blanket, wrapping the lungs, and helping protect the airways from attack.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The airways are lined by tiny hairs called cilia, which beat in an upward motion, to bring mucus up out of the airways. Noxious chemicals in tobacco smoke paralyse these cilia, and this contributes to stagnant pools of mucous, increasing susceptibility to infection.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Normally our bodies produce 1\u20131.4L of mucous per day, but we are generally unaware of it.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"cough-and-sputum\">Cough and sputum<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.nhsinform.scot\/illnesses-and-conditions\/lungs-and-airways\/cough\">A cough<\/a> is a natural body reflex. You cough occasionally, if something tickles the back of your throat, such as dirt or dust. This is perfectly normal.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If however you develop a more persistent cough, you may be coughing up sputum. This is called a productive cough. Coughing up sputum may be a sign of lung disease.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"what-are-the-causes-of-a-productive-cough-sputum\">What are the causes of a productive cough\/sputum?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A cough can be short term, usually under 3 weeks duration, or chronic.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A cough lasting less than 3 weeks is self-limiting. If it lasts longer than this you should go and see your doctor.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A <a href=\"https:\/\/www.nhsinform.scot\/illnesses-and-conditions\/lungs-and-airways\/cough\">chronic cough<\/a> may be due to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Upper respiratory tract infections, e.g. colds, influenza, viral infections, laryngitis, sinusitis.<\/li>\n\n\n\n<li>Asthma \u2013 a chronic lung condition caused by allergy.<\/li>\n\n\n\n<li>Lower respiratory tract infections, e.g. Pneumonia, Bronchitis.<\/li>\n\n\n\n<li>Bronchiectasis<strong> <\/strong>\u2013 a condition of widening of the airways.<\/li>\n\n\n\n<li>Chronic obstructive pulmonary disease (COPD)<strong> \u2013 <\/strong>smoking related lung disease.<\/li>\n\n\n\n<li>Drugs, e.g. Angiotensin Converting Inhibitors (ACE inhibitors) such as ramipril, lisinopril, used to treat high blood pressure\/ heart failure.<\/li>\n\n\n\n<li>Heart failure.<\/li>\n\n\n\n<li>Lung cancer.<\/li>\n\n\n\n<li>Pulmonary embolus.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"does-looking-at-your-sputum-help-make-a-diagnosis\">Does looking at your sputum help make a diagnosis?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.verywellhealth.com\/what-is-sputum-2249192\">Examination of your sputum<\/a> can give a lot of information about your condition.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"what-is-the-colour-and-consistency-of-your-sputum\">What is the colour and consistency of your sputum?<\/h3>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>Yellow\/green <\/strong>\u2013 contains white cells and suggests infection such as <a href=\"https:\/\/www.asthmaandlung.org.uk\/conditions\/pneumonia\" title=\"\">pneumonia<\/a>. Pneumonia may be caused by bacteria, or by viruses.<\/li>\n\n\n\n<li><strong>Brown \u2013 <\/strong>may due to tar staining in smokers, or due to blood in the sputum, may be seen in people with occupational chest diseases such as coal mining and pneumoconiosis.<\/li>\n\n\n\n<li><strong>Pink frothy sputum \u2013 <\/strong>can occur in heart failure.<\/li>\n\n\n\n<li><strong>Blood in the sputum <\/strong>\u2013 a common presentation in lung cancer, and in patients with blood clots in the lungs \u2013 pulmonary emboli.<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"what-happens-if-your-sputum-is-cultured-in-the-laboratory\">What happens if your sputum is cultured in the laboratory?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A sample of sputum can be <a href=\"https:\/\/www.healthline.com\/health\/routine-sputum-culture\">cultured<\/a> (grown)&nbsp; in the laboratory to see if there are bacteria present. The lab can then do further tests to see which antibiotic the bacteria are sensitive to.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"why-is-sputum-examined-under-the-microscope\">Why is sputum examined under the microscope?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A sputum sample can be examined under the microscope to look for lung cancer cells. This is called <a href=\"https:\/\/www.verywellhealth.com\/sputum-cytology-2249193\">sputum cytology<\/a>. Sputum can also be examined to look for evidence of <a href=\"https:\/\/www.healthlinkbc.ca\/healthlinkbc-files\/sputum-tuberculosis-testing\">tuberculosis<\/a> (TB).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"when-should-you-see-a-doctor-with-a-cough\">When should you see a doctor with a cough?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">You should <a href=\"https:\/\/www.nhsinform.scot\/illnesses-and-conditions\/lungs-and-airways\/cough\">see a doctor<\/a> if your cough:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Has lasted more than 3 weeks.<\/li>\n\n\n\n<li>Is severe, or getting worse.<\/li>\n\n\n\n<li>You are coughing up blood, have chest pain, or shortness of breath.<\/li>\n\n\n\n<li>You have other symptoms such as unexplained weight loss, voice changes, or lumps in your neck.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Coughing up blood is a <a href=\"https:\/\/www.nhs.uk\/conditions\/coughing-up-blood\/\">medical&nbsp;emergency<\/a>.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Even if it is just a few streaks of blood \u2013 you must see your GP without delay.<\/strong><\/li>\n\n\n\n<li><strong>Phone 999 if you are coughing up a lot of blood, or feel short of breath or have chest pain.<\/strong><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"how-to-manage-a-cough\">How to manage a cough<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The first step to <a href=\"https:\/\/www.nhs.uk\/conditions\/cough\/\">treating a cough<\/a> is to find a diagnosis. Your doctor will ask questions and examine you. You may be asked to blow into a machine \u2013 called a <a href=\"https:\/\/www.nhs.uk\/conditions\/peak-flow-test\/\">Peak Flow Meter<\/a> \u2013 to test your lung function. You may need a <a href=\"https:\/\/www.healthline.com\/health\/chest-x-ray\">chest X-Ray<\/a>, or an <a href=\"https:\/\/www.nhs.uk\/conditions\/electrocardiogram\/\">ECG<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Once you have a diagnosis, it is important to manage the underlying condition as well as possible.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"coughs-due-to-viral-infections\">Coughs due to viral infections<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Most coughs are due to <a href=\"https:\/\/www.healthline.com\/health\/acute-upper-respiratory-infection\">viral infections<\/a> from infections in the upper airways. Most commonly these are <a href=\"https:\/\/www.healthline.com\/health\/respiratory-syncytial-virus-rsv\">respiratory syncytial viruses<\/a>, or <a href=\"https:\/\/www.healthline.com\/health\/cold-flu\/parainfluenza\">parainfluenza viruses<\/a> for example. It\u2019s very important to note that antibiotics do not have any effect on viral infections. <strong>The National Institute for Care and Excellence (NICE)<\/strong> have produced <a href=\"https:\/\/www.nice.org.uk\/guidance\/indevelopment\/gid-ng10116\">2019 recommendations<\/a> &#8211; based on up-to-date published medical research, about the treatment of cough.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">They have stated clearly that with viral upper respiratory tract infections:<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-style-default is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">antibiotics make little difference to a persons\u2019 symptoms.<\/p>\n<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\">Furthermore they have said &#8211;<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">Antibiotic resistance is a huge problem and we need to take action now to reduce antibiotic use. Taking antibiotics when you don\u2019t need them puts you and your family at risk of developing infections which in turn cannot be easily treated.<\/p>\n<\/blockquote>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">These new guidelines will support GPs to reduce antibiotic prescriptions and we encourage patients to take their GPs advice about self-care.<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"how-to-self-care-with-a-cough\">How to self care with a cough<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">You can get help in assessing your cough by clicking on the <a href=\"https:\/\/www.nhsinform.scot\/self-help-guides\/self-help-guide-cough\">NHS self-help guide: Cough<\/a>.<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Stay well hydrated \u2013 this will help keep mucous thin and be easier to cough.<\/li>\n\n\n\n<li>Have hot, steamy showers as this helps to loosen mucous and clear sinuses.<\/li>\n\n\n\n<li>Avoid irritants such as perfumes, air fresheners, or cigarette smoke. There is so much help available, and now has never been a better time <a href=\"https:\/\/www.nhs.uk\/live-well\/quit-smoking\/nhs-stop-smoking-services-help-you-quit\/\">to give up smoking<\/a>.<\/li>\n\n\n\n<li>Try a <a href=\"https:\/\/www.nhs.uk\/conditions\/decongestants\/\">decongestant<\/a> \u2013 these aim to reduce inflammation in your nasal passages and dry up nasal secretions. Examples include <a href=\"https:\/\/www.healthline.com\/health\/allergies\/pseudoephendrine-vs-phenylephrine\">phenylephedrine and pseudoephedrine<\/a>. These can be purchased as tablets, liquids&nbsp; or nasal sprays. They can cause raised blood pressure, so should be used with caution if you have high blood pressure or heart disease.<\/li>\n\n\n\n<li>Try&nbsp; a cough suppressant containing <a href=\"https:\/\/medlineplus.gov\/druginfo\/meds\/a682492.html\">dextromethorphan<\/a> \u2013 but only if you are coughing a lot at night.<\/li>\n\n\n\n<li>Try an expectorant. These work by which thinning out the mucus. Look for an expectorant&nbsp; containing <a href=\"https:\/\/medlineplus.gov\/druginfo\/meds\/a682494.html\">guaifenesin<\/a>.<\/li>\n\n\n\n<li><a href=\"https:\/\/www.verywellhealth.com\/the-benefits-of-pelargonium-89023\">Perlagonium<\/a> is a herbal remedy for which there is some <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3183890\/\">evidence of efficacy<\/a> in treating the common cold. It has been shown to induce and support an immune response.<\/li>\n\n\n\n<li>Try honey! <a href=\"https:\/\/www.nhsborders.scot.nhs.uk\/media\/945401\/Leaflet-Cough-is-an-antibiotic-really-necessary.pdf\" title=\"\">The NHS now recommends honey<\/a> \u2013 and not antibiotics \u2013 for treating cough. In three published randomised clinical trials, children with respiratory infections given honey and lemon drinks, coughed less frequently at night.<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"final-thoughts\">Final thoughts<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Most of us are unaware we have sputum \u2013 until something goes wrong. Living with a cough is most unpleasant. The vast majority of coughs are due to viral infections, usually in the upper airways and much can be done to treat these at home without the unnecessary use of antibiotics.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you do cough, and this lasts for more than 3 weeks, do go and see your doctor. A correct diagnosis means you can access the best treatment for your underlying condition.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For example, asthma is a common cause of cough and is often under-recognised. Your doctor can help make this diagnosis. This is an allergic condition which responds to the use of <a href=\"https:\/\/www.asthma.org.uk\/advice\/inhalers-medicines-treatments\/inhalers-and-spacers\/\">asthma inhalers<\/a>.
